Transcript Request

Do you need transcripts for a continuing education training you attended (including VESi participants) at no extra cost?

• Download the Transcript Request Form with Adobe Reader 
• Complete the form and save it as a PDF
• Email the document to ceuprograms@sjsu.edu

It is recommended that attendees seeking CEUs transcripts should submit their request at least 4 weeks prior to the date needed.

For faster delivery, please choose e-transcript and provide an email address where the document can be sent. 

Undergraduate or graduate academic records at SJSU will not appear on this Continuing Education Units (CEUs) transcript. Academic transcripts must be requested through the SJSU Registrar's Office.

Have you already completed the training and need to pay for your CEUs?

• Provide the required course and personal information 
• Add to cart and check out to complete the request

A non-refundable 2.56% service fee will be charged by the third party if paid by credit card. We accept Visa, MasterCard, American Express, and Discover. Debit card payments will also have a non-refundable service charge. Avoid this charge by opting for ACH by providing your bank and account information.

Upon verifying attendance and grade rosters with your course facilitators, your CEUs request will be processed and delivered in 2 to 6 weeks.
